Before treatment:

The patient drank inferior liquor 3 years ago and his vision disappeared 3 days later.  5 days later, his vision had some restoration. 20 days later, he totally lost his left vision. Right vision was intact, but it was very poor. The color vision of both eyes disappeared. He received therapy in a local hospital, but the vision had no improvement. Before the treatment, the patient's left eye had no light sensation, right eye had light sensation. The patient could see fingers from 20 cm. He could see the fingers move from 40cm, he could see person's outline from 1 meter.

From the onset of disease, the patient had normal spirit. His weight had no obvious change. He had normal diet. The sleep quality was not good. He had normal defecation and urination. The patient has been smoking for many years and smokes 10 to 20 cigarettes. He drinks little liquor.

Admission PE:

Bp: 119/76mmHg; Hr: 50/min, Br: 20/min. Temperature: 36.7 deg. The patient's development and nutrition were normal. The thorax was symmetrical; the respiratory sounds of both lungs were clear, with no dry or moist rales. The heart sounds were strong, the rhythm was regular, and there was no obvious murmur in the valve area. The abdomen was flat and soft, with no masses. Through palpation, the liver and spleen were found to be normal. There was no swelling in both lower limbs. The ECG showed he suffered from sinus bradycardia.

Nervous System Examination:

Sergey Chirkov was alert and his speech was clear. His memory, orientation and calculation ability were almost normal. The diameter of both pupils was 5.0mms. Left eye had no response to light stimulus and right eye had slow response to light stimulus. He was unable to finish the examination of visual chart. Left eye had no light sensation. Right eye had light sensation and could see the fingers from 20cm. He could see the arm moving from 40cm. He could see the object's outline from 1 meter. The color vision of both eyes had disappeared. Both eyes could move flexibly to each side. There was no obvious nystagmus. Under the ophthalmoscope: the fundus presented with orange. The color of the optic nerve was pale. Arteriovenous proportion was 1:3. The forehead wrinkle pattern was symmetrical, the nasolabial sulcus was equal in depth, the teeth were symmetrical and the tongue was centered in the oral cavity. There was flexible movement in the neck. The muscle tone of all four limbs was almost normal; the muscle strength of all four limbs was about level 5. The tendon reflex of both upper limbs was elicited normally. The abdominal reflexes and tendon reflex of both lower limbs were weakened. The sucking reflex, palm and jaw reflex were negative. Bilateral pathological character was negative. The deep sensation and superficial sensation were normal. The coordinated examination was normal. There was no meningeal irritation sign.

Treatment:

We gave Sergey Chirkov a complete examination. He was diagnosed with optic atrophy and sinus bradycardia. He received treatment to improve the blood circulation in order to increase the blood supply and nourish the damaged neurons. He also received treatment to boost his immunity, antithrombosis and increase the heart rate.

Post-treatment:

After 4 weeks of treatment, the patient's vision has shown improvement. The patient's left side has light reflex, and the right eye has a more sensitive reflex than before. Right eye can distinguish fingers from 50cm and can see moving arm from 1 meter. He can see the outline of person from 2 meters.
